Submit Your Story Via Email

If you are unable to submit your story using our web form, you may submit your story via email to: info@nlmfoundation.org

Please include the following information in the email:

• First name
• Last name
• Country
• Participant’s email address
• Scanned copy of a signed AATG Consent Form (required from all participants)
• Scanned copy of a signed AATG Photo/Video Release (required from participants who submit photos and/or videos with their story).
